Medical weight loss

Weight-loss care that reads your labs, not just your scale.

Medical weight loss is a metabolic workup, not a fast GLP-1 script. Clara reads your whole chart against the label, then writes the on-label branded medication.

I want to lose weight. My BMI is 32. Can you put me on Ozempic?
Clara
Your BMI is 32.1 and A1C is 5.6% — on-label for Wegovy, not Ozempic. I'd start at 0.25 mg weekly and titrate to 2.4 mg maintenance. Sending this, plus a flag on your LDL of 148, to our physician.

How it works

Chart in, eligibility checked against the label, branded medication sent to your pharmacy, trend tracked weekly.

1

Full metabolic read, not a 3-question quiz

Clara auto-syncs your records from 150,000+ doctors' offices, hospitals, labs, and wearables, and reads your BMI, A1C, lipids, TSH, liver enzymes, sleep data, and medication list before recommending anything. Free to start, no credit card.

2

The right treatment, matched to the label

If a GLP-1 fits, Clara picks the molecule approved for your indication: Wegovy or Zepbound for weight management, Ozempic or Mounjaro if you also have type 2 diabetes. Behavioral, nutrition, and strength-training plans attach automatically. A licensed physician signs off on prescriptions.

3

Trend tracked, labs on-clock

Clara watches your weight trajectory, Oura or Fitbit data, home glucose if you have a CGM, and GI side effects, and titrates dose on the trend rather than a 90-day calendar. Repeat labs at 3 months are ordered automatically, not when you remember.

On-label eligibility read from your whole chart, not a three-question intake.

Eligibility

Who's a candidate for medical weight loss?

GLP-1 pharmacotherapy is appropriate when the FDA label fits.

BMI ≥30 (obesity), on-label for Wegovy and Zepbound34.

BMI ≥27 (overweight) with a weight-related comorbidity: hypertension, type 2 diabetes, or dyslipidemia34.

Type 2 diabetes with A1C above goal: Ozempic or Mounjaro, often paired with metformin.

Moderate-to-severe obstructive sleep apnea in adults with obesity: Zepbound carries an OSA indication4.

Treatments

The full toolkit, not just the drug with the biggest ad budget.

Rx · GLP-1 · Weight label

Wegovy (semaglutide 2.4 mg)

Once-weekly injectable semaglutide, titrated to 2.4 mg/week. FDA-approved for chronic weight management in adults with BMI ≥30 or BMI ≥27 plus comorbidity, and in adolescents aged 12 and older with obesity3.

BMI ≥30
or ≥27 with comorbidity
Rx · GLP-1/GIP · Weight label

Zepbound (tirzepatide)

Once-weekly dual GIP/GLP-1 agonist, titrated to 15 mg/week. FDA-approved for chronic weight management, and separately for moderate-to-severe obstructive sleep apnea in adults with obesity4.

OSA ✓
Zepbound is indicated for moderate-to-severe OSA
Rx · Adjunct

Metformin

Not FDA-approved for weight management, but commonly used off-label as an adjunct, especially in insulin-resistant patients and in PCOS. Typically stacked under a GLP-1 when type 2 diabetes coexists.

Protein, sleep, strength training

GLP-1s reduce appetite but also accelerate lean-mass loss if you don't protect it. Clara builds a plan around protein targets, resistance training, and sleep, and pulls in Oura, Fitbit, or Garmin data to track recovery and HRV alongside your weight trend.

Weight is a symptom of a metabolic story. Clara reads the whole story.

The reason weight-loss storefronts optimize for a fast GLP-1 script is that it's the easiest unit of revenue. Real medical weight management asks harder questions: Is your TSH drifting? Is your sleep apnea driving your cortisol? Is your resting heart rate up because the dose is too high? Is your LDL trending the wrong way while your weight comes down? Clara sees all of it.

Full-chart eligibility check

Clara runs BMI, comorbidity status, thyroid family history, pancreatitis history, pregnancy plans, medication interactions, and renal function against the Wegovy and Zepbound labels in seconds. A 15-minute visit cannot do this end-to-end.

Titration against the trend

Clara watches the real-world trajectory: weight week-over-week, wearable resting HR, sleep stages, GI symptom frequency. Step-ups happen when the pattern says to, not when the calendar says to.

The stuff a scale won't tell you

Clara flags lean-mass loss risk, falling LDL paradoxes, TSH drift on semaglutide, and rising resting heart rate early. Then it pulls in protein targets, resistance training, and labs on-clock. Your weight is one number in a metabolic picture that needs the other ten.

Common questions

Weight-loss questions patients actually search for.

What does Clara cost?
It's free to connect your medical records and start chatting with Clara. If you want Clara to provide medical advice, prescribe medications, or order labs, plans start at $25/month. All plans are HSA/FSA eligible. The cost of GLP-1 medication and lab work is separate and may be covered by your insurance, unlike the cash-pay model Hims and Lemonaid use for GLP-1s, which keeps the medication off your health plan entirely.
Am I a candidate for a GLP-1 if I'm not diabetic?
If your BMI is ≥30, or ≥27 with a weight-related comorbidity such as hypertension, type 2 diabetes, or dyslipidemia, you meet the FDA-labeled criteria for semaglutide (Wegovy) and tirzepatide (Zepbound)34. Clara reads your BMI, comorbidities, labs, and medication list before picking one.
What's the difference between Ozempic and Wegovy for weight loss?
They're the same molecule (semaglutide) but different FDA indications and different top doses. Ozempic is approved for type 2 diabetes (max 2 mg/week). Wegovy is approved for chronic weight management (max 2.4 mg/week). If weight is the goal and you're not diabetic, Wegovy is the on-label semaglutide. Insurance is far more likely to cover the product prescribed on-label for your condition.
What about muscle loss on a GLP-1?
Any large-magnitude weight loss pulls some lean mass with it. Clara flags lean-mass loss risk, builds in protein targets (roughly 1.2–1.6 g/kg of ideal body weight, adjusted for renal function), and pulls strength-training recommendations into the plan. If you use an Oura Ring, Fitbit, or Garmin, Clara integrates recovery and HRV into the picture.
What if a GLP-1 doesn't fit me?
GLP-1s are not always the answer. Patients with a personal or family history of medullary thyroid cancer or MEN 2, a history of pancreatitis, or planned pregnancy should not be on a GLP-1. Clara flags those up front and surfaces alternatives: metformin (off-label for weight), lifestyle/behavioral programs, or bariatric evaluation at higher BMIs. For moderate-to-severe obstructive sleep apnea with obesity, Zepbound carries an OSA-specific indication that's worth considering4.
Is Clara a real medical practice?
Yes. Clara is licensed in all 50 states. Diagnoses, prescriptions, and lab orders are reviewed and signed off by licensed physicians. Clara's AI handles intake, continuous monitoring, and follow-up; the clinical sign-off happens at the end.
How is this different from a weight-loss clinic or a DTC brand?
Can your old doctor read every lipid panel, every A1C, every TSH, and every wearable sleep score you've ever had before deciding whether a GLP-1 is the right call? Clara can. A weight-loss storefront runs a narrow intake, then ships you a monthly injection; it does not see your lipid panel drifting, your TSH change on semaglutide, or your resting heart rate climbing on Oura. Clara is full primary care, with a longitudinal view that weight-loss DTC cannot match by design.
Can I use my insurance, HSA, or FSA?
Clara's membership itself is cash-pay, not billed through insurance, but every plan is HSA/FSA eligible so you can pay it with pre-tax dollars. What the membership puts in motion may be covered by your insurance: prescriptions Clara writes go to your pharmacy and may be covered by your plan like any other script, and Clara deliberately prescribes branded FDA-approved Wegovy or Zepbound (not cash-pay compounded semaglutide/tirzepatide) so they stay eligible for standard insurance coverage. Ad-hoc lab orders Clara places may also be covered.
What is my data used for? Is it private?
Clara is HIPAA compliant. Your records are used to personalize your care. They're not sold, and they're not used to train public AI models.
What if I need in-person care?
Clara refers you to an in-person provider when a physical exam, imaging, or in-person procedure is needed. For weight management specifically, that includes bariatric surgical evaluation at higher BMIs, sleep studies for suspected OSA, and cardiology workup if there's established cardiovascular disease.
Can I use Clara for other health issues too?
Yes. One membership covers full primary care. The same chat handles your blood pressure, cholesterol, thyroid, annual labs, sick-day questions, and longevity work. That's the whole point, and it's what a single-condition weight-loss brand cannot offer.
