Drug and supplement interactions

Wegovy delays gastric emptying and can alter absorption of co-administered oral medications. Co-administration with insulin or sulfonylureas in patients with type 2 diabetes increases the risk of hypoglycemia, often requiring a dose reduction. Concomitant use with other GLP-1 receptor agonists is not recommended. Clara reads your full medication list before drafting.

Dose sizing

Per the Wegovy prescribing information, the standard escalation is 0.25 mg weekly × 4 weeks, then 0.5 mg × 4 weeks, then 1 mg × 4 weeks, then 1.7 mg × 4 weeks, then 2.4 mg weekly maintenance. Each step is for tolerability rather than weight effect; Clara watches your side-effect log against the calendar.1

On-schedule monitoring

Clara prompts a weight and BP recheck at 1 month and 3 months after initiation, then every 3 months. Renal function is monitored during dose escalation and with volume-depletion symptoms. Patients with established CV disease have lipid and CV risk re-evaluation on schedule.

Boxed warning

In rodent studies, semaglutide caused thyroid C-cell tumors. It is unknown whether Wegovy causes such tumors in humans. Wegovy is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Warnings

Acute pancreatitis (discontinue if suspected). Acute kidney injury (especially with dehydration from GI side effects). Hypoglycemia when used with insulin or sulfonylureas in patients with T2D. Acute gallbladder disease. Increased heart rate. Suicidal behavior and ideation (monitor for new or worsening mood changes). Hypersensitivity reactions. Most common adverse reactions are nausea, diarrhea, vomiting, constipation, abdominal pain, headache, and fatigue.

Monitoring requirements

Weight, BMI, and blood pressure at baseline and at scheduled follow-up intervals. Renal function during dose escalation and with volume-depletion symptoms. Heart rate (Wegovy can cause modest resting-HR increases). Mental-health check-ins for emergence of depression or suicidal ideation.

Special populations

Pregnancy: Wegovy is contraindicated in pregnancy due to potential fetal harm; discontinue 2 months before a planned pregnancy. Lactation: not recommended (limited data). Pediatric: approved for chronic weight management in patients 12 years and older with obesity (BMI ≥95th percentile). Geriatric: no overall dose adjustment, increased monitoring is reasonable. Renal/hepatic impairment: no dose adjustment recommended.

Can I use my insurance, HSA, or FSA?
Clara's membership is cash-pay and not billed through insurance, but every plan is HSA/FSA eligible. Prescriptions Clara's physicians write go to your pharmacy and may be covered by your plan like any other script — Clara prescribes FDA-approved branded and generic medications, not compounded formulations. Ad-hoc lab orders may also be covered by your insurance when applicable.

Both are semaglutide, the same molecule, made by Novo Nordisk. Wegovy is FDA-approved for chronic weight management (and for MACE risk reduction in people with established CVD and obesity or overweight); Ozempic is FDA-approved for type 2 diabetes. Different indications, different dose strengths at the upper end, different insurance coverage rules. Clara reviews your diagnoses and prescribes the on-label brand for your situation.
Does Clara prescribe branded Wegovy or compounded semaglutide?
Clara's physicians prescribe FDA-approved branded Wegovy, not compounded semaglutide. Branded Wegovy goes through the standard pharmacy fill and may be covered by your insurance. Compounded semaglutide is a cash-pay product sold by some telehealth brands; Clara does not use it because the FDA has issued safety concerns about compounded GLP-1 formulations and because branded coverage through insurance is better long-term economics for most patients.
How long do I stay on Wegovy?
Wegovy is intended for long-term chronic weight management. Stopping Wegovy is generally associated with regaining lost weight over time. Clara monitors your weight, BP, BMI, and tolerance throughout, and your physician reviews continuation, dose adjustments, or off-ramps at every visit. Some patients taper or discontinue once a weight-related comorbidity has resolved; this is an individualized decision.
