2:14am. My Oura resting HR has been over 70 for 9 nights. Should I be worried?
Clara
Your resting HR is up 18% and sleep efficiency dropped since the raise closed. That fits sympathetic load, not cardiac. Cut caffeine after noon for 10 nights; we'll escalate with our doctor if it holds.

Most founders don't have a doctor. They have a primary care slot they booked in 2022, an annual physical they've skipped twice, and a drawer full of half-finished prescriptions. Healthcare is a meeting that costs you half a day and ends with a nurse triaging you into a queue that moves on hospital time, not company time.

So the real healthcare gets run out of Slack. "Is anyone's Oura showing high resting HR this month?" "What's the deal with tirzepatide?" "My sleep score cratered, and my memory feels weird — is this burnout or something else?" You trade screenshots of lab PDFs and dose your own omeprazole.

Clara is the version of that thread that actually reads your chart. It auto-syncs records from 150,000+ sources, watches your wearable trajectory, and answers substantive clinical questions in seconds. It is AI-native primary care that will tell you, clearly, when the actual answer is "this needs an in-person workup."

What founders actually ask Clara

The healthcare-by-Slack playbook, replaced by something that reads your full chart.

2:14 am · Before a board meeting

"I think I have acid reflux but I also can't tell if it's a heart thing."

Clara reads your lipid panel, BP trend, family history, and current meds. Catches that you've been on ibuprofen for six months, which is almost certainly driving the reflux. Drafts a plan: stop NSAIDs, start omeprazole for 8 weeks, re-evaluate.

OutcomeGoing back to sleep in 20 minutes, not 3 hours.
On a Tuesday red-eye

"I think this is a UTI. I land in Austin in 5 hours."

Clara takes the intake in chat, confirms non-complicated features, and routes antibiotics to a pharmacy near your hotel. No urgent-care walk-in in a strange city, no waiting until Monday. See the UTI workflow.

OutcomePicking up the prescription on the Lyft to the hotel.
18 months in, demo day rising

"My Oura resting HR is creeping up. Is this stress or something real?"

Clara cross-references wearable trajectory, your last lipid panel, weight trend, and sleep efficiency. Spots a 14% lift in resting HR correlated with a 42-minute drop in sleep across 6 weeks. Frames it as a sympathetic-load pattern with a follow-up cadence, not an "everything is fine" platitude. Full chart integration with Oura, Garmin, and Eight Sleep.

OutcomeA plan with a 2-week checkpoint, not a 6-month PCP wait.
Year two, labs finally drawn

"My LDL is 162, my family has a cardiac history. What do I actually do?"

Clara reads ApoB (if drawn), Lp(a), the three prior lipid panels you have in scattered records, and your weight trend. Drafts a recommendation: atorvastatin 20 mg at night with an 8-week ApoB recheck. Escalation path pre-built for when you're not at target.

OutcomeGetting a real plan before you're 50, not at your third post-MI visit.

The 2am question belongs in a doctor's chat, not your group DM.

How it works

Healthcare on founder-time, not hospital-time.

1

Connect your chart

Clara auto-syncs your records from 150,000+ doctors' offices, hospitals, clinics, labs, and wearables. Including the labs you had drawn at a UCSF visit in 2022 that you've since lost the PDF for.

2

Ask at 2am

Instant clinical answers, with your full chart in context. The question you were about to text your doctor friend goes to an AI that can see more of your history than they can.

3

Physician sign-off where it matters

Prescriptions and lab orders are reviewed and approved by a licensed physician before they leave. FDA-approved branded or generic meds, eligible for insurance reimbursement at your pharmacy.

4

Pick up where you left off

Clara remembers the 6-week follow-up that's due on the stress-induced HR drift, the 8-week statin recheck, the next-due colonoscopy. You don't have to.

Founder-shaped primary care

The conditions that actually show up between Series A and Series C.

Stress & sleep

Burnout, insomnia, elevated resting HR

Clara cross-reads your wearable signal with chart context to tell burnout from something more mechanical. Sleep hygiene, short-course pharmacology where appropriate, referral if warranted. Integrates with Oura and Eight Sleep.

GI & GERD

Acid reflux you've been papering over for 18 months

The coffee-plus-stress combination that quietly produces chronic GERD. Clara drafts an 8-week acid-suppression course, triggers for endoscopy, and a titration plan. See acid reflux.

Cardiometabolic drift

LDL, ApoB, A1C, and weight

The year-3 founder lipid panel tells a story. Clara reads it against your wearable data and weight trend and drafts a real preventive plan — statin if warranted, GLP-1s if clinically appropriate. See high cholesterol, hypertension.

Acute & travel

UTIs, sinus infections, pink eye, travel medicine

Clara takes the intake in chat and routes a prescription to the nearest in-network pharmacy — even if you're in Austin, Lisbon, or halfway up a mountain. See UTIs and URIs.

A category of work a 15-minute PCP visit cannot do.

Founders aren't underserved by primary care because their conditions are unusual. They're underserved because the format doesn't fit their schedule and the 15-minute visit doesn't fit the data. An AI can.

Full-chart reasoning, at the speed of a Slack reply

Can your old doctor read your entire medical chart and your wearable trajectory and your prior labs every time you ask them something? Clara does, in seconds. That's a category-of-work difference, not a time-management difference.

Patterns across months of data

The 14% resting-HR lift correlated with a 42-minute sleep drop across 6 weeks is a pattern a 15-minute visit cannot surface. An AI looking at your Oura export and your chart together surfaces it as the first observation of the thread.

Continuity that survives your schedule

A founder's calendar destroys human continuity of care. Clara remembers the 8-week statin recheck, the 2-week stress-pattern follow-up, the next-due labs, and pings you inside the chat when they're due.

Your chart, your wearable, your question, answered instantly.

Founder FAQ

The questions founders actually ask before signing up.

Is this actually primary care, or is it a triage bot?
It's primary care. Clara's AI does the clinical reasoning end-to-end, and Clara manages chronic conditions (hypertension, cholesterol, type 2 diabetes, thyroid, GERD, asthma), handles acute issues (UTIs, URIs, rashes), and orders labs. It replaces the 15-minute visit; it refers you out when a physical exam, imaging, or procedure is required.
What if I travel a lot?
Clara operates through affiliated professional corporations licensed in all 50 states. The AI is always in your pocket; prescriptions are routed to whatever in-network pharmacy is closest. For international travel, Clara can still answer substantive clinical questions; prescribing is limited to US jurisdictions.
How is this different from One Medical?
One Medical is $199/year, plus visits billed to your insurance. The substantive work happens in scheduled visits, with copays and deductibles. The on-demand surfaces are wait-gated. Clara inverts the format: the substantive work happens instantly inside the AI chat. Clara's membership is cash-pay, HSA/FSA-eligible, not billed through insurance; the prescriptions Clara writes and the ad-hoc labs Clara orders may still be covered by your insurance at your pharmacy or Labcorp/Quest. See the full comparison on Clara's AI primary care page.
What does it cost?
Free to connect records and chat. Paid plans: $25/month Basic, $50/month Standard (wearable integration + 74 biomarker tests included), $150/month Concierge (108 biomarker tests included, unlimited messaging with medical team). All plans HSA/FSA eligible. Prescriptions and ad-hoc labs may be covered by your insurance when applicable.
Can Clara prescribe GLP-1s or ADHD meds?
Clara prescribes FDA-approved branded or generic GLP-1 medications (Ozempic, Wegovy, Mounjaro, Zepbound) where clinically appropriate — not compounded formulations. ADHD stimulants are a controlled substance and are outside current Clara scope; for ADHD care, Clara will help you navigate to a specialist.
Is Clara a real medical practice?
Yes. Clara operates through affiliated professional corporations licensed in all 50 states. A licensed physician reviews and signs off on diagnoses, prescriptions, and lab orders. Clara's AI handles the intake, clinical reasoning, longitudinal tracking, and follow-up.
What is my data used for? Is it private?
Clara is HIPAA compliant. Your records are used to personalize your care. They are not sold and not used to train public AI models. If you're a founder, your chart is at least as sensitive as your cap table.
