Output is flat but my heart rate is higher than it used to be at the same effort.
Clara
Matched-effort HR climbed from 146 to 159 bpm, and resting HR is up 8 bpm over 8 weeks. On metoprolol, the opposite would be expected. Ordering TSH, CBC, and ferritin for our doctor.

How it works

Connect Peloton. Clara reads your cardio data against your chart.

1

Connect Peloton and your records

Clara pulls your Peloton history (output, total work, heart-rate zones, resting HR, workout frequency) and auto-syncs your medical records from 150,000+ doctors' offices, hospitals, clinics, and labs. No PDFs, no faxed labs. Free to start.

2

Clara reads cardio drift, not just calories

Because the bike and tread are standardized, heart-rate-at-effort is unusually clean as a cardiovascular fitness signal. Clara cross-references it against your medications (beta blockers, thyroid meds, stimulants), recent labs, and blood pressure, and flags the inflections that map to something clinical.

3

Chat, labs, or Rx when warranted

Ask Clara anything. When the cardio pattern points at something treatable (medication effect, thyroid shift, anemia, worsening lipids, deconditioning), a licensed physician reviews Clara's plan and signs off on lab orders or prescriptions. FDA-approved prescriptions and ad-hoc lab work may be covered by your insurance.

What Clara watches

The Peloton signals a clinician almost never reads.

HR at effort

Heart rate-at-effort drift

A steady output with climbing heart rate across matched-effort intervals is a genuine cardiovascular signal. It tracks with subclinical thyroid disease, anemia, dehydration, poor sleep, new stimulants, alcohol, and, in older adults, evolving cardiac issues. Clara reads it against your medication list and recent labs before labeling it "deconditioning".

Matched effort
Clara compares like rides, not averages
Resting HR

Resting HR trend across non-ride days

Peloton's resting HR across non-ride days is one of the cleanest blood-pressure-adjacent signals most patients have. A multi-week climb, especially on a stable medication regimen, is a flag. Clara reads it against your home BP cuff and your most recent chem panel.

8 weeks
Rolling window Clara reviews against BP & labs
Output trend

Unexplained output decline

A sustained drop in output at a consistent training cadence, without lifestyle explanation, is a real clinical signal. Iron depletion, hypothyroidism, poorly-controlled hypertension, and early cardiac issues all show up here before the annual physical. Clara queues the right workup.

Medication effects

Cardio response to a new medication

Starting a beta blocker, a statin, a GLP-1, or an SSRI visibly changes Peloton data. Clara knows which changes are expected and which are not. A beta blocker should flatten HR-at-effort; a GLP-1 should reduce output only if you're undereating. Clara distinguishes expected pharmacology from a genuine clinical shift worth flagging.

The fitness data your doctor ignores, read as signal.

Peloton's history is probably the richest standardized-effort cardio dataset a typical patient owns. Same bike, same tread, same metrics, multiple times per week, for years. A human clinician has neither the time nor the tooling to read it. Clara reads every ride against your chart, your meds, and your last lab panel, and treats cardiovascular fitness the way it actually should be treated: as a vital sign.

Standardized effort is a clinical gift

Unlike free-form running, Peloton's bike and tread give a controlled effort where heart-rate-at-power is comparable across months. That makes HR-at-effort drift one of the cleanest signals a consumer wearable produces. Clara treats it accordingly.

Medication-aware cardio reading

Starting a beta blocker, a GLP-1, a statin, or an SSRI visibly changes what your Peloton shows. Clara, with the full medication list open, distinguishes expected pharmacology from a genuine clinical shift, and catches the latter before the annual visit.

Cross-reads cardio against your labs and chart

Cardiorespiratory fitness is one of the strongest predictors of all-cause and cardiovascular mortality in adults. Clara reads your Peloton output, HR-at-effort, and resting HR against your lipid panel, fasting glucose, TSH, iron studies, and blood pressure, and turns that into a preventive primary-care plan. Common questions

Peloton with Clara: the questions patients actually ask.

What does Clara cost?
Connecting your medical records and chatting with Clara is free. Wearable and connected-device integration (Peloton, Oura, Eight Sleep, Garmin, Fitbit, Whoop, Apple Watch) is included with the Standard plan at $50/month, which also includes 74 biomarker tests as part of the membership (not billed separately, not billed to insurance). All plans are HSA/FSA eligible. Prescriptions Clara writes and ad-hoc lab orders are billed by your pharmacy or lab and may be covered by your insurance.
How does Clara use my Peloton data?
Clara pulls your Peloton workout history (output, total work, heart-rate zones, resting HR, workout frequency) and reads it against your full medical chart: current medications, recent labs, prior diagnoses, and anything else on file. When a cardio pattern intersects with something clinical, Clara surfaces it and queues the right next step.
My heart rate at effort is climbing. Is that a problem?
A rising HR-at-effort against a stable output is a real signal, especially on a standardized bike or tread. The common drivers are subclinical thyroid disease, anemia, dehydration, poor sleep, a new medication, caffeine/stimulant shifts, and, in older adults, worsening blood pressure or evolving cardiac issues. Clara reads your trend against your chart and queues labs (often TSH with reflex free T4, CBC, ferritin, BMP, lipid panel) when the pattern deserves it. For condition-specific context, see hypertension or hypothyroidism.
Is Clara a real medical practice?
Yes. Clara is licensed in all 50 states. All diagnoses, prescriptions, and lab orders are reviewed and signed off by licensed physicians. Clara's AI handles intake, continuous monitoring, and clinical reasoning.
How is this different from my regular doctor?
Can your old doctor read every Peloton ride you've done this year against your last lipid panel and your medication list, every time you meet? Clara can. A 15-minute annual physical sees a snapshot; Clara reads the trajectory continuously and queues the right workup when the cardio trend says something the annual physical won't catch. For broader primary-care context, see AI primary care.
Can I use my insurance, HSA, or FSA?
Clara's membership is cash-pay, not billed through insurance, but every plan is HSA/FSA eligible so you can pay it with pre-tax dollars. Prescriptions Clara writes go to your pharmacy and may be covered by your plan like any other script; ad-hoc lab orders at Labcorp or Quest may be covered when applicable. The 74-test biomarker panel bundled into the Standard plan is a separate category. That cost is included in the membership itself and is not billed to your insurance.
Is my Peloton and medical data private?
Clara is HIPAA compliant. Your Peloton data and medical records are used to personalize your care. They are not sold, and they are not used to train public AI models.
