My A1C is 8.1 on metformin. Is Mounjaro a fit?
Clara
Your A1C is 8.1% despite metformin 1000 mg BID, with CGM showing post-lunch spikes above 210. No MTC or pancreatitis on file, so Mounjaro is on-label. I'd add 2.5 mg weekly alongside metformin; sending to our physician.
How often will you re-check my A1C?
How it works

Chart in, eligibility checked against the Mounjaro label, branded medication sent to your pharmacy, A1C trend watched quarterly.

1

Connect your records and CGM

Clara auto-syncs your records from 150,000+ doctors' offices, hospitals, clinics, labs, and wearables. The AI reads your A1C trend, fasting glucose, eGFR, thyroid and pancreatitis history, and current anti-hyperglycemics against the Mounjaro label before anything is prescribed.

2

Prescription to your pharmacy

A licensed physician signs the Mounjaro prescription and it's sent to your pharmacy. Branded Mounjaro may be covered by your insurance — unlike the cash-pay compounded tirzepatide DTC weight-loss brands push.

3

Titrate to your glucose trend

Mounjaro titrates 2.5 → 5 → 7.5 → 10 → 12.5 → 15 mg in 2.5 mg increments no sooner than every four weeks1. Clara watches your A1C, CGM time-in-range, and GI tolerability so step-ups happen when the chart says they should.

Dosing

The Mounjaro ladder, 2.5 mg at a time.

Rx · Initiation

2.5 mg weeks 1–4

Titration dose only. Mounjaro 2.5 mg is not effective for glycemic control but is required to introduce the molecule and reduce GI side effects before therapeutic dosing begins1.

4 wks
Then 5 mg
Rx · Step-up

5 → 7.5 → 10 mg

Each 2.5 mg step can occur no sooner than four weeks apart. Clara watches A1C velocity, CGM time-in-range, and nausea frequency between each step and recommends slowing if your trend flattens at a lower dose.

3 steps
4 weeks each
Rx · High dose

12.5 → 15 mg weekly

Maximum maintenance dose. In SURPASS-2, adults taking 15 mg achieved A1C reductions up to 2.4% at week 401. Most patients don't need to titrate this high; Clara stops escalating once your A1C is in target range.

Metformin (usually retained)

Most patients starting Mounjaro stay on metformin. It's the first-line T2D agent in guidelines, cheap, insurance-friendly, and mechanistically complementary to tirzepatide. Clara only deprescribes it for renal or GI reasons.

T2D treatment is a lifetime game. Mounjaro without continuous monitoring is a gamble.

Type 2 diabetes doesn't stop when the prescription is sent. You need the A1C trend read every quarter, the eGFR checked, CGM patterns interpreted, and the dose titrated against all of it. A 15-minute quarterly visit cannot do that. An AI reading your chart every time you message it can.

A1C trend, not a snapshot

Clara reads your entire A1C history alongside CGM time-in-range, GMI, and fasting glucose. A single A1C of 7.3% can mean different things depending on whether you were at 8.1% last quarter or 6.9%.

Renal function watched continuously

Mounjaro requires dose adjustment caution in renal impairment and has reports of acute kidney injury, often from dehydration secondary to GI side effects1. Clara watches eGFR trends and GI tolerability between every labs panel.

On-label for the indication you have

If you have T2D, Mounjaro is the right script. If weight is the primary indication and you don't have T2D, Zepbound is the on-label product. Same molecule, different label; Clara matches the label to your chart.

One AI that reads SURPASS, your CGM trend, and your full chart on every dose decision.

Common questions

Mounjaro questions patients actually search for.

What does Clara cost?
It's free to connect your medical records and start chatting with Clara. If you want Clara to provide medical advice, prescribe medications, or order labs, plans start at $25/month. All plans are HSA/FSA eligible. The cost of Mounjaro and lab work is separate and may be covered by your existing insurance, unlike the cash-pay model most GLP-1 telehealth brands use.
Am I eligible for Mounjaro?
Mounjaro is FDA-approved to improve glycemic control in adults with type 2 diabetes, as an adjunct to diet and exercise1. Eligibility hinges on a T2D diagnosis and ruling out contraindications: personal or family history of medullary thyroid carcinoma or MEN 2, prior serious hypersensitivity to tirzepatide, and caution with history of pancreatitis, severe GI disease, or diabetic retinopathy. Clara reads all of this from your chart before drafting anything.
What's the difference between Mounjaro and Zepbound?
Same molecule (tirzepatide), different FDA-approved indications. Mounjaro is on-label for type 2 diabetes. Zepbound is on-label for chronic weight management in adults with obesity, and separately for moderate-to-severe obstructive sleep apnea with obesity4. If weight is the indication and you don't have T2D, Zepbound is the correct script and much more likely to be covered.
How does Mounjaro compare to Ozempic?
Ozempic (semaglutide) is a GLP-1 receptor agonist. Mounjaro (tirzepatide) is a dual GIP/GLP-1 receptor agonist. In SURPASS-2, tirzepatide produced larger A1C and weight reductions than semaglutide 1 mg at doses of 5, 10, and 15 mg1. Whether one is "better" for you depends on your chart, your insurance formulary, and how you tolerate each. See our full comparison at Ozempic and GLP-1s.
Will Mounjaro also help me lose weight?
Mounjaro is not FDA-approved for weight loss — that's Zepbound. But T2D patients on Mounjaro do frequently lose clinically meaningful weight as a secondary effect. If your primary concern is weight rather than diabetes, Clara will steer you to Zepbound on-label rather than prescribe Mounjaro off-label. See the weight loss page for the full picture.
Can I stay on metformin while taking Mounjaro?
Yes — and for most patients, you should. Metformin is first-line in T2D guidelines, mechanistically complementary to GLP-1 agents, insurance-friendly, and safe for long-term use. Clara only deprescribes it for GI intolerance or significant renal impairment.
Is Clara a real medical practice?
Yes. Clara is licensed in all 50 states. Diagnoses, prescriptions, and lab orders are reviewed and signed off by licensed physicians. Clara's AI handles intake, continuous monitoring, and follow-up; the clinical sign-off happens at the end.
How is this different from seeing an endocrinologist?
Can your endocrinologist read your full chart, your CGM every day, your wearable resting HR and sleep, and your recent lipids and BP every time they see you? That's Clara's job. Clara refers you to an in-person endocrinologist when a procedure or physical exam is needed — but the continuous chart-reading, ongoing titration, and daily questions are where the AI earns its keep.
Can I use my insurance, HSA, or FSA?
Clara's membership itself is cash-pay, not billed through insurance, but every plan is HSA/FSA eligible so you can pay it with pre-tax dollars. What the membership sets in motion may be covered by your insurance: Mounjaro prescriptions go to your pharmacy and may be covered by your plan, and labs Clara orders may also be covered. Clara does not sell compounded tirzepatide.
What is my data used for? Is it private?
Clara is HIPAA compliant. Your records are used to personalize your care. They're not sold, and they're not used to train public AI models.
Can I use Clara for my blood pressure and cholesterol too?
Yes. One membership covers full primary care, not just diabetes. The same chat handles hypertension, cholesterol, thyroid, annual labs, and longevity work. Diabetes rarely travels alone; treating it in isolation is a mistake.
