Drug and supplement interactions

Mounjaro delays gastric emptying and can alter absorption of co-administered oral medications. Co-administration with insulin or sulfonylureas increases the risk of hypoglycemia, often requiring a dose reduction of the concomitant drug. Tirzepatide may reduce the efficacy of oral contraceptives at therapy initiation and after dose escalation; the label recommends a non-oral backup contraceptive method for 4 weeks after initiation and after each dose increase.

Dose sizing

Per the Mounjaro prescribing information, the starting dose is 2.5 mg subcutaneously once weekly for 4 weeks (for tolerance, not glycemic control), then 5 mg weekly. Dose may be increased in 2.5 mg increments after at least 4 weeks at the current dose, up to a maximum of 15 mg weekly.1

Boxed warning

In rodent studies, tirzepatide caused thyroid C-cell tumors. It is unknown whether Mounjaro causes such tumors in humans. Mounjaro is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Warnings

Acute pancreatitis (discontinue if suspected). Acute kidney injury (especially with dehydration from GI side effects). Hypoglycemia when used with insulin or sulfonylureas. Diabetic retinopathy complications in patients with a history of retinopathy. Acute gallbladder disease. Hypersensitivity reactions. Reduced efficacy of oral contraceptives at initiation and dose escalation. The most common adverse reactions are nausea, diarrhea, decreased appetite, vomiting, constipation, abdominal pain, and dyspepsia.

Monitoring requirements

A1C at baseline and every 3 months until at goal. Renal function during dose escalation and with volume-depletion symptoms. Patients on insulin or sulfonylureas should be monitored for hypoglycemia. Patients with a history of diabetic retinopathy should have ophthalmic monitoring.

Special populations

Pregnancy: discontinue Mounjaro when pregnancy is recognized; tirzepatide may cause fetal harm based on animal data. Lactation: limited data; consider risks and benefits. Pediatric: safety and effectiveness in pediatric patients have not been established. Geriatric: no overall dose adjustment, but increased monitoring is reasonable. Renal/hepatic impairment: no dose adjustment recommended.

Both are tirzepatide, the same molecule, manufactured by Eli Lilly. Mounjaro is FDA-approved for type 2 diabetes; Zepbound is FDA-approved for chronic weight management in adults with obesity (or overweight with a weight-related comorbidity) and for moderate-to-severe obstructive sleep apnea with obesity. Insurance coverage and pricing differ by indication. Clara reviews your diagnosis and chart and prescribes the on-label brand for your situation.
Does Clara prescribe branded Mounjaro or compounded tirzepatide?
Clara's physicians prescribe FDA-approved branded Mounjaro, not compounded tirzepatide. Branded Mounjaro is dispensed through the standard pharmacy fill and may be covered by your insurance. Compounded tirzepatide is a cash-pay product sold by some telehealth brands; Clara does not use it because the FDA has issued safety concerns about compounded GLP-1 formulations and because branded coverage through insurance is better long-term economics for most patients.
What if I'm getting side effects from Mounjaro?
