GLP-1 medications we prescribe
Prescription only. In studies in rodents, semaglutide, tirzepatide, and orforglipron caused thyroid C-cell tumors. It is not known whether they cause these tumors in people. Do not use if you or a family member has ever had medullary thyroid carcinoma (MTC), or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). The most common side effects are nausea, diarrhea, vomiting, constipation, and abdominal pain. See full Important Safety Information.

Boxed warning: risk of thyroid C-cell tumors. In studies in rodents, semaglutide, tirzepatide, and orforglipron caused thyroid C-cell tumors. It is unknown whether these medications cause thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans. These medications are contraindicated in patients with a personal or family history of MTC and in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Warnings and precautions. Acute pancreatitis, including fatal and non-fatal hemorrhagic or necrotizing pancreatitis; discontinue if pancreatitis is suspected. Acute gallbladder disease. Acute kidney injury due to volume depletion, especially with dehydration from gastrointestinal side effects. Severe gastrointestinal adverse reactions. Hypersensitivity reactions, including anaphylaxis and angioedema. Hypoglycemia, particularly when used with insulin or a sulfonylurea. Diabetic retinopathy complications in patients with type 2 diabetes. Increased heart rate. Pulmonary aspiration during general anesthesia or deep sedation; tell your care team you are taking one of these medications before any scheduled surgery or procedure. Never share an injection pen between patients, even if the needle is changed.
Most common adverse reactions. Nausea, diarrhea, vomiting, constipation, abdominal pain, dyspepsia, abdominal distension, injection site reactions, fatigue, headache, dizziness, eructation, gastroesophageal reflux disease, flatulence, and hair loss.
Drug interactions. Tirzepatide and orforglipron may reduce the effectiveness of oral hormonal contraceptives because of delayed gastric emptying. Patients using oral contraceptives with tirzepatide should switch to a non-oral method or add a barrier method for 4 weeks after starting and for 4 weeks after each dose increase. Patients using oral contraceptives with orforglipron should switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase.
Pregnancy and lactation. These medications are not recommended during pregnancy. Discontinue semaglutide at least 2 months before a planned pregnancy. Discontinue any of these medications when pregnancy is recognized. There are limited data on use during breastfeeding; discuss the risks and benefits with your clinician.
